In adherence with the American Society for Reproductive Medicine guidelines, clinics and agencies require donors and gestational carriers to have an objective personality test as part of the qualification process. This test assists a clinician in screening for mental disorders and other psychological problems that could negatively interfere with the process. The test may also provide information to help determine if the donor or carrier's self-report answers were honest.

What Is the PAI Plus?The PAI Plus (Personality Assessment Inventory – Plus) is a psychological questionnaire used to help evaluate a sperm donor’s emotional and mental health. It’s a standard part of the screening process for many fertility clinics and agencies to ensure that donors are emotionally well and suitable for donation.
The PAI is a widely used objective inventory of adult personality that assesses psychological functioning and potential psychopathology. It provides a comprehensive overview of clinical syndromes, interpersonal styles, and potential risk factors. Scoring and reporting are completed using a secure digital platform called PariConnect.
In the context of sperm or egg donation, the PAI is used as a screening tool to support mental health professionals and fertility clinics in evaluating a candidate’s psychological suitability for donation. The assessment helps identify traits or conditions that may be important to consider, such as emotional stability, impulsivity, substance use tendencies, and interpersonal functioning. While the results are not used to diagnose, they assist in ensuring that donors meet the psychological criteria necessary to move forward in the process in a responsible and ethical way.
Why It Matters in Donor Screening
When someone chooses to donate sperm, their emotional well-being can play a role in the long-term success of the donation process—for the donor, intended parents, and any future child. The PAI Plus gives a fuller picture of the donor's psychological functioning and helps inform thoughtful, ethical decisions.
This is not a test of intelligence or personality type (like a Myers-Briggs or enneagram). Instead, it helps a licensed professional look for any potential concerns related to emotional stability, stress management, decision-making, or behavioral health.
